    By signing this document, I am acknowledging that I have read and understood the listed Terms and Conditions.

    I acknowledge that I have signed up to attend Next Level Basketball Ltd.

    Payment of fees

    I acknowledge that any outstanding fees owed to Next Level Basketball Ltd must be settled immediately. I acknowledge that all fees requested for payment will be paid in full and on time.

    Cancellation, Refunds and Swapping sessions

    I acknowledge that once I have registered and paid, a refund will only be provided in the following circumstances:

    • The participant has a medical condition which prevents them from any further activity (A doctor’s certificate will be required)
    • At Next Level Basketball Ltd sole discretion

    I acknowledge that if I cannot commit to the same day for any of the camps or trainings (Private Training, One on One, School Coaching, Training Camp and/or Holiday Camp etc), then I will choose the pay by session option which allows for flexibility through all the days and sessions available.

    Please Note:

    We do not offer refunds or make-up sessions for the following:

    • Missed sessions
    • Late notice (More than 24 hour’s notice of cancelation)
    • Change of plan
    • Not showing up

    Risk

    I acknowledge basketball is a contact sport in which physical injury may occur from time to time. Next Level Basketball Ltd does not hold any responsibility for any injuries that may take place during training camps.

    Bullying and behaviour

    Next Level Basketball Ltd is committed to creating a safe environment where students can enjoy participation in our activities and programs. Next Level Basketball Ltd has zero tolerance for bullying and/or harassment. The consequence for this behaviour is immediate exclusion from activities. This decision is made at the coach/business’ discretion.

    Privacy and use of images

    Next Level Basketball Ltd respects and acknowledges your information is private. We will not share your personal information with any party.

    I agree to being contacted by the Next Level Basketball Ltd coaches and staff if required.

    I acknowledge and consent to the use of photographs and/or videos being taken of the participant, participating in basketball activities.

    I acknowledge and agree that such photographs and/or videos may be used on social media and for promotional purposes. If I do not want photographs and/or videos taken of the participant, it is my responsibility to inform Next Level Basketball Ltd via email.

    Medical Conditions

    I acknowledge it is mine/ the caregiver’s/participant’s responsibility for the participant’s health and I am required to inform Next Level Basketball Ltd of any medical conditions and/or allergies. If the participant requires inhalers or an EpiPen to be nearby due to a medical condition, I acknowledge it is my responsibility to provide this and make our coaches aware of this.
